Lower Nicola Indian Band

Urgently hiring a Full / Part time Community Health Nurse

$36–$50 an hour

HEALTH PROMOTION/EDUCTION:

· To identify health care needs of the community and to facilitate preventative health care programs in response to those needs.

· To plan and conduct regular meetings and workshops on a range of topics promoting and understanding of health care and safety and the prevention of illness, injury and disease.

· To teach groups/individuals of all ages ways to achieve and maintain good health in the areas on nutrition, AIDS awareness, diabetes awareness and prevention,

· To bring health conditions which may require intervention to the attention of the appropriate health service provider (e.g. CHN or HCN Referral)

· To establish strong partnerships with other community agencies and service providers on and off the reserve

· Acts as a liaison with other LNIB Community Services programs on behalf of clients

· Performs home visits with elders, chronic clients, and pre/post natal clients

DIABETES:

· Assist communities to plan and deliver culturally appropriate educational activities related to awareness of diabetes, its prevention, self-management care and treatment

· Co-ordinate information, advice and support for people with diabetes

· Encourage community members to participate in more physical activity

· To deliver up to date information and distribute through workshops, pamphlets and newsletters

FIRST NATION HEALTH BENEFITS (FNHB):

· To assume responsibility for and delivery of Patient Travel for on reserve, status Indians

· To provide FNHB services, support and advocacy for client via telephone, walk-in or appointments

· To maintain current updated guideline information and rates for delivery of the program

· To ascertain the status of clients and their eligibility for funding under the FNHB

EDUCATION / CERTIFICATION / EXPERIENCE

• Bachelor's Degree (required)
• related nursing: 5 years (preferred)
• BC Nursing Certification (required)
