Role Summary
As a Community Support Worker, you will be a valued member of our home care team, supporting clients in maintaining their independence and well-being within their own homes. You’ll report to the Care Coordinator and follow personalized care plans tailored to each client's needs.
Key Responsibilities
- Assist with meal preparation and light housekeeping
- Provide companionship and emotional support
- Accompany clients on outings, errands, and appointments
- Support clients with ambulation and transportation
- Offer medication reminders
- Accurately complete documentation/charting
- Respect and follow individual care plans
Qualifications
- ? Clean Criminal Background Check (or willing to obtain one)
- ? Negative TB skin test or chest x-ray
- ? Access to reliable transportation
- ? Internet and smartphone access
- ? Experience in home support or caregiving (an asset, but not required)
