Is looking for a Social Worker Navigator – Outreach
$36.00-$45.00 an hour
NLX Health is seeking a compassionate and collaborative Health & Wellness Navigator who is a Registered Social Worker (RSW) to join our interdisciplinary team. This role supports wraparound care by improving access and coordination across mental health and addictions, wellness outreach, and clinical services. The Social Worker will work closely with internal teams and report directly to the Psychiatric Nurse Manager.
Key Responsibilities:
- Serve as the primary outreach and navigation contact for community members seeking health and wellness services
- Conduct holistic assessments and guide individuals through available supports including mental health clinicians, nurses, physicians, and other wellness programs
- Support individuals experiencing mental health and substance use challenges by applying culturally safe, trauma-informed approaches aligned with the First Nations Health Authority (FNHA) Mental Wellness continuum framework
- Coordinate care with FNHA-funded services and ensure access to appropriate treatment, harm reduction supports, and wellness resources
- Collaborate with internal teams to ensure timely referrals and culturally safe care coordination
- Maintain accurate documentation and contribute to reporting requirements under the JPB agreement
- Participate in community outreach and engagement activities to promote service awareness and accessibility
- Travel with the Nlaka’pamux territory
Qualifications:
- Registered Social Worker (RSW) in good standing with the BC College of Social Workers
- Clinical training and experience in mental health and/or substance use support
- Strong understanding of Indigenous health and wellness frameworks
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently and collaboratively in a multidisciplinary environment
- Valid Class5 driver’s license and access to a reliable vehicle
- Legally authorized to work in Canada as a Canadian citizen or permanent resident
